In order to obtain underwritten quotes for groups with 2 – 50 employees, we will need a census form completed with employee names and dates of birth for employees and dependents. If there are current group benefits in place it is helpful to have copies of the current benefit summaries.
For groups with 50 or more employees, a group health statement with 2 years of claims experience needs to be completed by someone in Human Resources. If the group does not have 2 years of claim experience then each employee will have to complete a Uniform Medical Assessment Form (UMAF’s) in order to obtain a group quote.
When traveling outside the U.S. you should carry travel insurance. This inexpensive insurance can save you not only in terms of money but also in terms or receiving quick, modern medical care. You can get a quote and enroll HERE in an International Medical Global plan that is right for you.

All individual plans offered from all insurance carriers fall into one of these categories. New Mexico insurance carriers participating in ACA plan offerings are BC/BS, New Mexico Health Connections, Molina (primarily a Centennial Care Plan) and Presbyterian.
